Graduation
Congratulations on your upcoming graduation from OHSU! To graduate in your desired term, you’ll first need to submit a graduation application.
Apply to graduate
To apply for graduation, log in to the Student Self-Service portal.
- Select Apply to Graduate.
- Review your program information. Important note: The term presented on this screen is NOT your graduation term — you will select your intended graduation term on the next screen. If your program information is correct, select Current Program then Continue. If your program information is incorrect, contact the Office of the Registrar at regohsu@ohsu.edu or 503-494-7800.
- Follow the instructions to select your graduation term and the name you’d like to appear on your diploma. You can apply to graduate up to three terms in advance.
- The application fee of $60 per degree will be applied to your student account under Payment Method.
- Review the information in your application summary. If everything is correct, submit your application. You should see a confirmation message. You can view your submitted application by selecting View Graduation Application from the Student Services menu.
- If you are in a dual program, complete the dual degree form to apply for your second degree.

Frequently asked questions
-
I missed the graduation application deadline. What should I do?
If you missed the current term’s application deadline, it’s still possible to graduate this term. Follow these steps:
- Apply for the next available term in Student Self-Service (even though it’s not the term you want).
- Complete the change graduation term form and select Move graduation one term earlier.
-
I need to move my graduation to a different term. Do I have to apply again and pay $60?
No, there is no charge to change your graduation term. Complete the change graduation term form to move your graduation either one term earlier or up to four terms into the future.
-
How do I cancel my graduation application?
Complete the change graduation term form and select Cancel graduation application.
However, if you still plan to graduate within four terms of the term you applied for, we recommend you select Move graduation application one to four terms into the future. Doing so will avoid the $60 fee to reapply through Student Self-Service in the future.
-
When will my degree/certificate be awarded?
Degrees and certificates are awarded only after all grades have been submitted and degree requirements have been verified.
-
What is the difference between graduation and commencement?
Graduation is the awarding of your degree or certificate after the verified completion of your program’s requirements.
Commencement is a ceremony to celebrate your achievement.
-
When will I receive my diploma?
We will mail your diploma six to eight weeks after the last day of your graduation term. At the same time, we’ll send an electronic copy to your OHSU student email address.
-
How do I update my address?
You can update your address in Student Self-Service, but only before the last day of your graduation term.
-
My legal name changed. How do I make sure my diploma has my new name?
Complete the name change form before the last day of your graduation term to update how your name will appear on your diploma. You will need to upload proof of identity, such as a driver’s license or marriage license.
