Enrollment and Degree Verifications

We strive to fulfill all enrollment verification request in 7 business days and your request will be added to our processing queue and processed in the order received.

    OHSU has authorized the National Student Clearinghouse to provide enrollment and degree verifications to third parties. The National Student Clearinghouse can be contacted:

    • Online at www.degreeverify.org
    • By mail at National Student Clearinghouse, 2300 Dulles Station Boulevard, Suite 220, Herndon, VA 20171 

    Enrollment Verification Forms:

     Please submit your form through our Enrollment Verification Request process :

    1. Input personal information
    2. Request: Completion of the attached form & upload form in PDF format & specify term to be verified
    3. Include any additional information if needed
    4. Deliver to: Input delivery instructions
    5. Sign & Submit

     

      Enrollment Letter:

      If you are in need of an enrollment verification letter, please submit one through our Enrollment Letter Request process:

      Specialized Verification Requests

      Please follow the steps below to request your documentation needed by the Oregon Medical Board:

      Order Deans Letter & Upload Form

      1. Log in or create an account in Parchment
      2. Service- Select ‘MSPE’
      3. Recipient- Select or input the Oregon Medical Board’s mailing or email address
      4. Attachment - An area to upload an attachment will appear. Upload OMB’s Education Verification form to the order. IMPORTANT: Make sure to complete your personal information portion of the form only
      5. Submit

      Please follow the steps below to request your documentation needed by the Medical Board of California:

      Official Transcripts:

      1. Log in or create an account in Parchment
      2. Service: Transcript
      3. Select Delivery Method: Electronic
      4. Select: Send to myself or an individual
      5. Recipient: Electronic – mbclicensingelectronicnotifications@mbc.ca.gov
      6. Finish all prompts
      7. Submit

      Certify My Diploma

      1. Log in or create an account in Parchment
      2. Service: Certify My Diploma
      3. Select: By Mail
      4. Select: Send to myself or an individual
      5. Recipient: Manually input “DOCS Portal”, then the MBC address below
      6. Finish all prompts
      7. Submit

      Med Form:

        1. Navigate to the Enrollment/Degree Verification Request Form
        2.  Provide the requested information
        3.  For type of verification, select “Completion of the attached form”
        4.  An area to upload a form will generate. Upload your form there. Please be sure to complete the Student Info. sections before submitting.
        5. Recipient Information – input DOCS PORTAL
        6. Submit

      By inputting “DOCS Portal” as the recipient, we will upload all your requested documentation directly into the portal.

      Please follow the steps below to request your documentation needed by the Texas Medical Board:

      Order Official Transcripts & Upload Form D:

      1. Log in or create an account in Parchment
      2. Service: Transcript
      3. Recipient: Input recipient address by selecting “Send to myself or another individual” AND input TX Medical Board Address (P.O. Box 2029, Austin, TX 78768-2029)
      4. Attachment: Upload your verification form with the applicant section completed and a photo attached
      5. Submit

      Schedule a Video Call (WebEx) with the Registrar's Office to verify your picture identity for your Form D – we will schedule this once all your form have been requested and completed on our end right before we mail them.
