Enrollment and Degree Verifications
OHSU has authorized the National Student Clearinghouse to provide enrollment and degree verifications to third parties. The National Student Clearinghouse can be contacted:

- By mail at National Student Clearinghouse, 2300 Dulles Station Boulevard, Suite 220, Herndon, VA 20171
OHSU students and alumni requesting an enrollment verification letter may use the Enrollment or Degree Verification Request Form. We strive to fulfill all enrollment verification requests in 7-10 business days.
If you need an education verification or degree verification form filled out, please first check if the form requires transcripts or any other documents to be sent with it. If the form does require these documents, please reach out to the Registrar’s Office through email at firstname.lastname@example.org for further instruction.
If it does not require additional documents, please submit your form through the Enrollment/Degree Verification Request Form. Your form will be added to our processing queue and processed in the order received.
