Enrollment and Degree Verifications

If you need to provide verification of your OHSU enrollment or degree for applications, professional licensing or other purposes, the Office of the Registrar can help. 

If you’re a third-party entity, we cannot fulfill your request for verification directly. Please use the National Student Clearinghouse verification services, which is authorized to provide verification for OHSU students’ enrollment and degrees. 

For residency or fellowship records, contact the Graduate Medical Education (GME) office.

Request enrollment or degree verification

If you’re a current or prior OHSU student, complete the enrollment or degree verification request form.  

  1. Provide required information, including your name, birth date, contact information and school.
  2. Choose one or more types of verification. Depending on your needs, we can:
    1. Provide an enrollment or degree verification letter
    2. Complete a verification document you attach
    3. Provide a specialized letter with information according to your instructions
  3. Request any additional information you would like us to include, such as your anticipated graduation date.
  4. Request delivery by email, mail, fax or online portal and enter the associated information in the fields that appear.
  5. Sign and submit the form. 

We try to fulfill all enrollment verification requests in seven business days. We process requests in the order received. 

Specialized verification requests

Oregon Medical Board

To provide a medical student performance evaluation (MSPE) to the Oregon Medical Board, complete the following steps.  

  1. Go to the Parchment website and enter your email address to log in or sign up.
  2. On your dashboard, select MSPE.
  3. For recipient, select or input the Oregon Medical Board’s mailing or email address.
  4. Attach the appropriate Oregon Medical Board form for education verification.
    1. IMPORTANT: Complete only the upper portion of the form — the rest must be completed by OHSU.
  5. Submit the order.

Medical Board of California

To provide required documentation to the Medical Board of California, complete the following steps. 

Official transcripts 

  1. Go to the Medical Board of California’s Third Party Services page and scroll to Medical School E-Transcripts. Follow the instructions to complete the form to get the transcript submission email address.
  2. Go to the Parchment website and enter your email address to log in or sign up.
  3. On your dashboard, select Transcript.
  4. For delivery method, select Electronic and select Send to myself or an individual. For recipient, input the transcript submission email address you got from the board.
  5. Submit your order. 

Certified diploma 

  1. Go to the Parchment website and enter your email address to log in or sign up.
  2. On your dashboard, select Certify My Diploma.
  3. For delivery method, select By Mail and select Send to myself or an individual. For recipient, input the words “DOCS Portal” then the Medical Board of California’s mailing address. Find the board’s mailing address on their Contact Us page.
  4. Submit your order. 

Medical Education Documentation (MED) form 

  1. Complete the enrollment or degree verification request form.
  2. Provide required information, including your name, birth date, contact information and school.
  3. For type of verification request, select Completion of the attached form. An area to upload your MED form will appear. Make sure to complete the applicant information sections first, then upload your form.
  4. For delivery method, select online portal. A field for Portal’s web address will appear. Simply input the words “DOCS Portal” and we will upload your requested documentation directly to the portal.
  5. Sign and submit the form. 

Texas Medical Board

To provide required documentation to the Texas Medical Board, complete the following steps. 

Form D – Dean’s Certification 

  1. Complete the enrollment or degree verification request form.
  2. Provide required information, including your name, birth date, contact information and school.
  3. For type of verification request, select Completion of the attached form. An area to upload your form will appear. Make sure to complete the applicant information sections first, then upload your form.
  4. Choose your delivery method and input the recipient information.
  5. Sign and submit the form.
  6. After you submit your form, we will schedule a WebEx video call with you to verify your identity. This call requires you to have a camera and will take about two minutes.
