About me

I’m Dr. David J. Mazur-Hart, a fellowship-trained neurological surgeon. I perform cranial, spinal, and peripheral nerve surgery. I have a special interest and subspecializes in treating the most challenging conditions of the spine such as deformity correction, scoliosis surgery, revision surgery and spinal tumors.

I completed my neurological surgery residency at Oregon Health & Science University and pursued additional training in complex spinal surgery and minimally invasive spinal surgery techniques at the University of California, San Francisco.

I partner with my patients to determine the best treatment plan that will help improve their quality of life. As a native Oregonian, I’m grateful to care for the great people of the Pacific Northwest. In my free time, I enjoy fly fishing, hiking, playing sports and spending time with family.

Insurance

Before scheduling an appointment

  1. Check your network. If you have health insurance,  call your company to find out if the OHSU Health location or provider you plan to visit is part of your network.
  2. Ask what you will pay. Your insurance company can tell you what your costs are likely to be.

If you schedule an appointment and your health insurance does not include OHSU Health, you may have to pay more than if you go to a provider in your insurance network.

Visit our billing and insurance page for more information.
