What you need to know about student compliance and immunization requirements 

All OHSU School of Nursing students are required to provide documentation regarding the completion of their immunization and compliance records as identified in the Student Compliance and Drug Screening policy 50-01.15 and the OHSU Student Health Service pre-entry guidelines policy 02-01.001. Review the following steps to complete these requirements by the stated deadline.

New students ordering background checks, drug screens: 

Please read before placing your background check and drug screen orders through Advanced Reporting: According to OAR polices (OAR-409-030-0220 background checks; OAR-409-030-0210 drug testing;),  new students cannot complete a background check and drug screen more than 90 days before the educational term begins. 

New students will be receiving an email around 90 days prior to matriculation that a background check and drug screen request has been submitted for you to a platform called Advanced Reporting. Advanced Reporting is our background check and drug screen platform partner. Please be on the lookout for an email from automation@instascreen.net . You will need to complete the requested information in that email to become compliant for the OARs background check and drug screen policies. An electronic authorization form will be emailed to you which will allow the background agency, Advanced Reporting LLC, to verify the personal information you provide, and share its results with SON the Compliance Officer. It’s important to note that once you receive the email from Advanced Reporting, you only have 14 days to complete both the background check and drug screen.

Continuing students that need an updated background check for a clinical rotation, please reach out to the compliance officer or clinical placement coordinator for further assistance. 

Ordering a closer drug screen location for students in more rural locations:

If you are not able to locate a close testing site for your drug screen, you may try and request a closer collection site, but you will need to reach out to Advanced Reporting directly for assistance.

NOTE: New incoming students gain access to their OHSU accounts, Compass and their network IDs 30 days prior to their start date. It is important for students to retain the email that contains account information as that email will expire. As a reminder, new students will not have access to complete their required Compass modules until 30 days prior to matriculation.

Criminal history may impact future nursing licensure and impact the ability of a student to progress through their designated nursing program.  Please review the Background Check Disclosure form below and turn in the signature form as part of your incoming compliance materials.

Submit all completed documents to Complio and Acemapp.

  • Review the list of required immunizations for your program in the instructional brochure.
  • COVID-19 Vaccination InformationOHSU requires immunizations for its students in accordance with recommendations from the Centers for Disease Control and Prevention (CDC) and from the State of Oregon. This means that OHSU now requires that all members, including students, be fully vaccinated against COVID-19 before starting their program. Students must comply with OHSU pre-entrance immunization requirements per Policy 03-30-150 and Policy 02-90-010, and remain in compliance for the duration of their enrollment at OHSU. This means that all admitted students must provide proof of full COVID-19 immunization or receive an approved medical or religious exception prior to the start of the academic program. Admission into OHSU academic programs are considered conditional until proof of full vaccination is provided or a medical or religious exception is approved by OHSU. If students have not met the COVID-19 policy 21 calendar days prior to the first day of their academic program, their conditional offer of admission to OHSU will be withdrawn and they will be unable to take courses or start their programs of study or other academic activities. ‘Fully vaccinated’ means an individual has received both doses of a two-dose COVID-19 vaccine, or one dose of a single-dose COVID-19 vaccine, and that at least 14 days have passed since the final dose of the COVID-19 vaccine was issued before starting the program. This definition may be modified to include subsequent boosters and/or additional doses as recommended by the CDC. If a new student would like to apply for an exemption for the COVID vaccination requirement, they will need to submit their exemption no more than 45 days prior to starting the program. If you have specific questions or concerns regarding this, please contact the SON compliance officer.
  • OHSU requires all eligible students to carry comparable medical health insurance. All eligible students will be automatically enrolled in the student health insurance plan (excludes RNBS) and have the student health insurance plan premium billed to their student account unless they have other comparable coverage AND complete and are approved for a SHIP waiver by the deadline. Students Medical insurance questions should be directed to OHSU's Business Office, 503-494-7617 Option 4.
  • Flu Attestation/Declination: Students taking courses in clinical agencies are required to get a flu shot prior to participation. Students that do not wish to get a flu shot can submit a completed Flu Shot Declination Form and upload it into their Complio for tracking purposes.

Licenses and certifications go into your Complio and Acemapp accounts.

  • CPR Certification (required of all students except public health and nursing undergraduate students). We can only accept CPR/BLS certifications from the American Heart Association, no exceptions. If in an undergraduate program, we are no longer making it a requirement for new undergraduate students to get their CPR/BLS certification before starting their program. New undergrad nursing students will get CPR/BLS certified in one of the nursing courses starting their first term in the program. Please note that students cannot be in clinicals if their CPR/BLS certification lapses. Non-compliant students will be pulled out of clinical if it lapses. Students are responsible for maintaining an active certification at all times while in their program. OHSU does offer a CPR/BLS certification renewal course. You can reach out to the Compliance Officer for more information.
  • Please check your campus specific information. 
  • Copy of RN license (not required for Public Health, Accelerated Bachelors, or 3 year-Bachelor of Science students) CRNA Students ONLY- Please provide documentation for ACLS, BCLS, and PALS. Students who are required to have an RN license for their program cannot be in clinicals if their RN license lapses. Students will be pulled out of clinical if it does laps. Students are responsible for maintaining an active RN license at all times while in their program if it is required. 
  • Electronic photo requirements for PDX Graduate and RNBS students. Accelerated Bachelors and 3 year Bachelors students on the Portland campus do not need to submit this form or a photo.

Load your documents and track your compliance. All School of Nursing students are required to remain in Compliant status throughout their academic experience at OHSU. Students will be pulled from clinicals and have a compliance registration hold placed on their student account for falling out of compliance. 

  • Log into Complio with your e-mail and password.
  • Provide all supporting documentation (certificates of completion, copies of trainings and certifications, forms, immunization medical records, etc) to Complio and Acemapp accounts.
  • Log into Acemapp with your OHSU email and password. You will need to provide all supporting compliance documentation here as well as Complio.
  • Accessing your Passport in Complio: Your Passport will show you your overall compliance along with what items have been approved/are compliant and what ones is non-compliant. You can access your Passport by clicking on the Report tab on the left side tool bar. This will open your "clinical passport". You will have the option to save this passport to your computer. Sometimes your faculty will ask you to provide your passport to them to make sure you are fully complaint. 

All new students are required to enter their COVID vaccination information into Complio and Enterprise Health (EH).

Students are required to enter their COVID information into two platforms: Complio and Enterprise Health (EH). If a student has received their COVID vaccination(s) outside of OHSU, they will need to enter their COVID vaccination information into an OHSU's platform called Enterprise Health (EH). Students will use their OHSU network ID to log in. If a student is experience log in issues with EH, they need to reach out to the SON compliance officer directly so they can troubleshoot. If a student receives their COVID vaccinations through OHSU, then their information should already be in EH, and the student will only need to upload their COVID vaccination information into their Complio account.

Once a student has access to their OHSU network ID and access to Compass, they will be able to log into Enterprise Health (EH). This is about 30 days before matriculation. If a student is experiencing log in issues with either of these items they will need to reach out to their program associate or the compliance officer so they can assist.

OHSU requires all admitted students to provide proof of full COVID-19 immunization or receive an approved medical or religious exception prior to the start of the academic program.

OHSU does not allow unvaccinated students to participate in in-person direct patient contact or in simulation activities involving in-person direct contact with standardized patients, and as such, entry into clinical programs for unvaccinated students will be prohibited.

Below are the steps and screenshots that will show students the process on how to upload their COVID information into the EH platform. 

  1. Log into Enterprise Health using your network OHSU ID information: https://webchartnow.com/ohsu/webchart.cgi
  2. Click on Upload COVID Vaccine Documents
  3. Upload your document and hit submit 

If students have not met the COVID-19 policy requirements 21 calendar days prior to the first day of their academic program, their conditional offer of admission to OHSU will be withdrawn, their conditional offer of admission to OHSU will be withdrawn and they will be unable to take courses or start their programs of study or other academic activities.

Renewing Complio

Annually you will be asked to renew your Complio account. There is no charge to renew. When you get to the payment screen, please select “invoice to institution”. 

Instructions on how to renew:

1. Click on “Order History” on the left side of the screen
2. Under ‘Purchased Subscription(s)’, you will see ‘Subscription Renewal’. 
3. Click on ‘Renew Subscription’
4. You will be provided with two options; Renew Subscription or Change Subscription. 

  • If you are needing to change your package type (ie. fall/spring, 2nd Year/3rd Year, etc.) click on ‘Change Subscription’ 
  • If no changes are needed, click ‘Renew Subscription’

5. Continue through the ordering process.

Please note: once your account is renewed or changed these changes may take a few minutes to reflect on your account.

Instructions on how to print your Complio passport: 

1. Log into your Complio account and click on the tab called "report" on the left hand side
2. After you click on report, this will open up a second window
3. Once the second window is open you will want to click on the blue floppy disk at the top and select PDF. This will automatically create a PDF document which is your passport.
