Biography

Improving Addiction Care Team (IMPACT):
Principal Investigator and Director

Dr Englander's work is at the intersection of addiction, health system transformation, and public health. She is the Principal Investigator and Director of the Improving Addiction Care Team (IMPACT) at OHSU. IMPACT is an interprofessional addiction medicine consult service with linkages to community addiction care for medically complex hospitalized patients with substance use disorders.  IMPACT started in July, 2015 and has served over 2400 individuals. Dr Englander leads a Substance Use Disorder in Hospital Care ECHO, a telementoring program that is supporting spread of hospital-based addictions care among interprofessional providers across Oregon hospitals. 

Care Transitions Innovation (C-TRAIN):
Co-Principal Investigator and Director

C-TraIn is a nationally recognized, multicomponent transitional care improvement program that was started for socioeconomically vulnerable adults at OHSU. C-TraIn includes 1) transitional nursing, social work, and pharmacy care; 2) enhanced linkages to primary care medical homes; and, 3) explicit efforts to better integrate care regional systems across hospital and community settings. Dr Englander co-founded C-TraIn at OHSU, and led expansion to three hospitals in the Legacy Health through funding from the Center for Medicare and Medicaid Innovation (CMMI).
