Credentials verification

•    Non-OHSU practitioners ordering outpatient treatment services must hold an active license in the state where they provide care to the patient and must be working within their scope of practice under state law.  

•    The receiving department staff will verify whether the prescribing provider is active by verifying the respective state licensure boards’ online provider credential verification website.  

•    If the provider’s current licensure cannot be verified, the receiving staff shall not accept the order or referral.
