OHSU’s Eugene Child Development and Rehabilitation Center evaluates and cares for children and young adults with developmental needs and chronic conditions.

Our team:
- Cares for patients up to age 21 (age 24 for Transition Clinic).
- Does outreach clinics 10 times a year in Medford and twice a year in Klamath Falls.
- Has longtime relationships with community providers.
- Works with partners to help children close to home:
- Oregon Center for Children & Youth with Special Health Needs, which helps families get low-cost health care and services.
- Care Coordination (CaCoon), which connects families with registered nurses for home visits.
When your child gets a referral to Eugene CDRC, you’ll need to fill out an intake packet:

Once we get your intake packet, we’ll put your child on a waiting list for an appointment. We must have a complete intake packet to add a child to our waiting list.
