How to Recover MyChart Username or Password
If you forget your OHSU MyChart username or password, select ‘forgot username’ or ‘forgot password’ located under the ‘sign in’ button (see red arrow).
Steps to recover username
If you forget your username, you will be asked to verify your first name, last name, date of birth and zip code. Once you click the submit button, an email from mychart@ohsu.edu will be sent to your email containing your MyChart username. The email subject will say “MyChart Verification code”.
If you do not know any of the information below or you are having trouble recovering your username, contact the MyChart help desk (503) 494-5252. Their hours are from 7:00 AM-6:00 PM Monday-Friday.
Steps to recover password
If you forget your password, you will need to verify your username, date of birth and zip code.
If you do not know any of the information below or you are having trouble recovering your password, contact the MyChart helpdesk (503) 494-5252. Their hours are from 7:00 AM-6:00 PM Monday-Friday.
You will then be asked to enter a one-time security code that is 6 characters long. The code can be sent as either a text message or email. The code is only valid for 30 minutes.
Enter the code that was sent to you in the ‘enter code’ box (see red arrow) and select ‘next’.
The final step is to create and verify (retype) a new password. Once you push the ‘next’ button, you will be taken to your homepage in your MyChart account.
