The Oregon Office of Rural Health (ORH) is searching for Oregon’s next Community Star. A Community Star is a person(s) or organization that makes a difference in our rural communities. We will announce the winner at the Annual Oregon Rural Health Conference this October, and they will join the other National Organization of State Offices of Rural Health (NOSORH) Community Stars on National Rural Health Day, celebrating the power of rural. 

The nominees must be:

  • Individuals, organizations or coalitions/consortia working or volunteering in a designated rural area. Check the Am I rural? tool to verify.

Not eligible:

  • Government officials who are elected or appointed to positions in the executive, legislative or judicial branches of state or federal government; or
  • Past Community Stars (past Community Stars are listed here).

To nominate a Community Star, please fill out this survey and submit it by July 20, 2026.

Explanation: I nominate Jane Doe as this year’s Community Star for Oregon. Jane is the community health coordinator at the Maplewood Clinic and has been chosen because of her remarkable dedication and impact on our community. Notably, she initiated a program that provides essential healthcare services and educational resources to underserved families in our area, and volunteers with our local EMS agency in her spare time. Jane’s tireless efforts in promoting rural healthcare access and wellness initiatives have been inspiring. Her leadership, compassion, and unwavering commitment to making a difference have left a mark on us all. Whether it's providing emergency health care to those in need or spearheading community initiatives, Jane embodies the spirit of service.
