Patients we treat

We provide primary care with a focus on adults ages 80 and older. Our team has specific training in care for seniors, so we can manage all of the health concerns that arise as you age. Your provider will help you access the services you need, when you need them.

Our services

Our primary care team provides age-friendly health care for older adults. Our providers also can draw on the expertise of OHSU’s geriatric specialists for consultations and treatment planning.

Our team offers care to help you maintain your health and meet your goals. We’re your first stop for checkups, screenings, minor illnesses and injuries, managing chronic illnesses and caring for conditions such as dementia.

When you need to see a different provider or go the emergency room, we’ll review your records and follow up.

Our team can help you:

  • Develop a plan to maintain or improve mobility.
  • Prevent, identify and manage dementia and related geriatric syndromes.
  • Make sure your medications support your well-being and do not contribute to falls, dementia or other health concerns.
  • Prioritize your personal goals as you grow older, and ensure that your wishes are followed for end-of-life care.
