Antimicrobial Stewardship

Our Antimicrobial Stewardship efforts span OHSU, the Portland VA, and Hillsboro Medical Center (HMC). The Antimicrobial Stewardship Program at each hospital is co-led by a physician (Dr. Ellie Sukerman - OHSU and HMC, Dr. Kristina Bajema - VA) and pharmacist team (James Lewis, PharmD- OHSU, Kim MacKay, PharmD- VA). We are committed to optimizing the use of antimicrobial medications with the goal of improving patient safety and outcomes as well as decreasing antimicrobial resistance.

Our efforts include:

  • Prospective audit and feedback
  • Maintenance of a restricted antimicrobial formulary
  • Development and dissemination of guidelines for antimicrobial use
  • Medication use evaluations
  • Implementation of IV to PO conversion protocols
  • Provider education across acute care, long-term care, and ambulatory settings
  • Mentorship of Infectious Diseases fellows, pharmacy residents, and other trainees
  • Development of a pharmacist-led antibiotic allergy testing consult service at OHSU
