Understanding pathways to support, future planning and resources for those with memory problems and their care partners

Led by clinical staff from the Layton Aging and Alzheimer’s Disease Center and a visiting representative from the Alzheimer’s Association, this one-visit educational session is for patients, family members and care partners who are new to Alzheimer's and related dementia.

Topics covered:

  • Caring for the person with a memory disorder
  • Support for the caregiver
  • Introduction to Alzheimer's Association programs
  • Community resources
