Welcome to the OHSU Physician Assistant Program Alumni and friends web page. From our first graduate in 1997 through our 500th (and beyond) in 2015, we would like you all to come back often to revisit and reconnect. Check out how to order transcripts, make a gift to our new endowment fund or find out more information on becoming a preceptor to the program. Stay in touch, we would love to hear from you!
To order an official transcript, please visit the Registrar's Office website and follow the instructions for online transcript ordering.
For unofficial transcripts or online access of your academic records, please log into OHSU's Student Information System. The Student Information System requires a valid UID number as well as a PIN. If you are unable to remember your UID, please contact the University Registrar at 503-494-7800. If you are unable to remember your PIN, please contact the Helpdesk at 503-494-2222.
Verify a degree. The National Student Clearinghouse is now our authorized agent for providing degree and enrollment verifications. If you are an employer or background screening firm requesting this information, please visit www.degreeverify.org. For forms requiring additional information (e.g., state licensure forms during your first year post-graduation), please email the PA Program at email@example.com. Please include a signed release with all requests.
Typhon data. Instructions on accessing your Typhon data Note that Typhon data is only available for 5 years after graduation.
Letters of recommendation. Requests for letters of recommendation can be e-mailed directly to your faculty advisor or to firstname.lastname@example.org. Please be advised, for students that have been out of the program for 5 years or more, professional letters of recommendation will be a generic description of program content and graduation requirements. We suggest using current work references for more up to date reference content. All letters of recommendation must include a signed release.
