Health Care Linkage Project

The Linkage Project is for adults with intellectual and developmental disabilities (I/DD) who are not getting their health needs met. There are two main goals of the Linkage Project:  

  1. The first goal is to help people learn about health issues that are important to them.  

  1. The second goal is to offer ways to help people be healthy and supports to help them get there.  

The project will happen for four years. It will start in Southwest Oregon and then move to other parts of the state.  

The Oregon Office on Disability and Health is looking for adults with intellectual and developmental disabilities (I/DD) who want help with using health care or being healthy. Linkage Project Team from the Oregon Office on Disability and Health will help in three ways:   

  1. They will ask people in the project questions about their health needs.  

  1. They will help people learn about healthy behaviors, going to the doctor, going to the dentist, getting screening tests, and getting mental health counseling.  

  1. They will help people find health care and other health supports in their community.
