Linkage Project: Information for Community Partners and Support Persons
The Linkage Project connects adults with intellectual and developmental disabilities (I/DD) with unmet needs to preventive health care and health promotion programs in their community. The Oregon Office on Disability and Health (OODH) Linkage Project Team is seeking referrals for individuals who may benefit from Linkage Project activities.
The Linkage Project is a 10-state Centers for Disease Control and Prevention (CDC) project and in Oregon it is carried out in collaboration with Oregon Office of Developmental Disability Services (ODDS).
Together with the participant and their chosen supports, the Linkage Project hopes to come alongside to focus on a targeted preventive health goal over 3 months. The Linkage Project provides resources like self-advocacy tools and support person fact sheets, to help remove barriers to health care. Our Linkage Team can assist participants in English and Spanish.
Linkage Project: Referral Guide for Community Partners and Support Person
1. The Community Partner/Support Person completes the Linkage Project Referral form on behalf or with potential Candidate.
2. The Linkage Project Team looks at the form and will contact the Community Partner/Support Person, or the Candidate, with any questions about the form.
3. Upon Linkage Project Referral form acceptance, the Linkage Project Team will contact the Candidate to set up a first meeting. The Linkage Project Team will explain the project and answer any questions during this first meeting.
4. The Linkage Project Team will work with the Participant for 3 months on personal health goals!
