OR-FACE Narratives

The Oregon Fatality Assessment and Control Evaluation (OR-FACE) team has written brief narratives that describe real fatal workplace accidents in Oregon. These narratives examine contributing factors and provide recommendations for prevention. We encourage you to use these narratives and other OR-FACE resources for educational purposes, to help save the lives of other workers in similar situations

OR-FACE is a research program; investigators do not enforce compliance with state or federal occupational safety and health standards and do not determine fault or blame.
