CaCoon (CAreCOOrdinatioN)

OCCYSHN contracts with local public health agencies to implement the CaCoon program, a public health nurse home visiting program. The program has operated in most Oregon counties for over 30 years. CaCoon is currently available in 25 counties in the state of Oregon. CaCoon home visitors help families coordinate care for their children and youth with special health needs.  Children and youth served in the CaCoon program make fewer visits to the emergency department. They also have higher rates of immunization, annual well-child, and dental care visits. CaCoon serves families of children and youth age 0-21. The most vulnerable families, such as those with a newly diagnosed infant, are prioritized for services.

CaCoon nurses partner with families to:

  • Identify the child's strengths and needs.
  • Connect with healthcare and other community supports.
  • Make sure the child’s health team works well together.
  • Help gather information to make health-related decisions for the child.
  • Identify problems and help find solutions.

    For youth with special health needs aged 12-21, some counties offer CaCoon services to prepare for the transition to adult health care, work, and independence.
