ADA Obesity and CKD Quality Improvement in 
Primary Care
Topic: Improve the capacity for primary care practices to support adult patients with diabetes. This program combines clinical education and quality improvement coaching to improve clinic practices and workflow to 1) improve patient-centered conversations around weight and supporting evidence-based obesity management and 2) improve screening and referrals for Chronic Kidney Disease (CKD). Currently recruiting primary care practices that serve adult patients with 
diabetes in Oregon, Washington, and Idaho.

PARADE — Parent-Assisted Remote Assessment of DErmatitis
Topic: PARADE Aim 2 is about how dermatologists evaluate eczema severity in young children. The study is recruiting parents of children aged 30 days to 5 years who have been diagnosed with eczema and receive care at OHSU. Study activities include a short survey about the child’s skin and eczema history and taking pictures of the child’s skin so that a Dermatologist evaluate the eczema remotely. Participants receive $25 at the completion of the study.

REACH — Real-world Evidence to Advance Multi Cancer Early Detection Health Equity (REACH/Galleri-Medicare study)
Topic: The national REACH study aims to see how a simple blood test can find cancer early in more than 50 types of cancers. Currently enrolling patients 50 years and older insured by Medicare or Medicare Advantage and cancer-free for at least three years. Enrollment locations include Astoria, Beaverton, Hood River, Portland, and The Dalles.

SMBP — The Self-Measured Blood Pressure Study 
Topic: To learn more about pharmacist support for high blood pressure. We are hoping to find out if working with both a pharmacist and a primary care team helps patients monitor and lower their blood pressure more effectively than working with only the primary care team. Currently recruiting primary care practices to support the enrollment of 20 adult patients 65 and older who have high blood pressure and one or more additional cardiovascular conditions.
