Consortium Programs

Make placement requests directly through ACEMAPP.

Non-Consortium Programs

Email the OHSU Clinical Placement Coordinator, Jennifer Hale, to confirm placement availability before coordinating ACEMAPP profile.

ADN, BSN and APRN placements

ACEMAPP is an online placement and credentialing system for all students/schools requesting ADN, BSN and APRN placements. OHSU uses ACEMAPP for placement requests, to verify student records, and to track and manage student placements on our campuses. We ask our education partners to submit placement requests via this system and to verify that all the student records, ex. drug screens, immunizations, criminal background checks, etc. have been successfully completed.    

ACEMAPP includes the attestation of the administrative requirement for ADN, BSN and APRN students.

To get started in ACEMAPP, please go to acemapp.org/contact and complete the “Contact Us” form with your information. Select “New Organization Account Access” as the subject and let us know in the message that you are intending to connect with OHSU Health System. You may also call 844-223-4292 to speak with an ACEMAPP team member.

BLS Certification Requirement

OHSU does not accept AHA RQI certification. Learners must obtain an AHA Basic Life Support (BLS) Provider certification card.

Please review the information required for a student to obtain an OHSU ID badge.
All Rotations: Background Check Attestation Letter
OHSU requires that all background checks are in alignment with Oregon Health Authority standards. A background check letter is required for a student to receive the required OHSU student ID badge.
Background check attestation letters must be on school letterhead with the current date, and include: Student name, School/University name, criminal background status, student standing, and signature of the Clinical Placement Coordinator or Faculty member.
Example: “To Whom it May Concern, (Student Name) has passed a criminal background check that includes Social Security number trace, State/National criminal background history, sex offender registry check, and OIG LEIE check, and is a student in good standing at (Name of School or University). Signed by: Clinical Placement Coordinator or Faculty.”
Non-nursing Rotations: Background Check Attestation Letter & Administrative Requirements Attestation
In addition to the background check attestation letter, for Medical Assistant and Certified Nursing Assistant rotations, each student is required to complete an attestation of administrative requirements. Attach both the background check attestation letter and the attestation of administrative requirements documentation to the Onboarding Request Form located in Step 4.

Attach the required documentation from Step 3 to the Onboarding Request Form. 

This should be completed at least four weeks before the placement start date. 

Please complete the form for each student. This form serves as a request for OHSU access and tracking for all visiting students. The information is used to request access for an OHSU ID badge, network, Omnicell, Epic, etc.  Please make sure that names are spelled correctly, all information is accurate, and indicate if the student has been placed at OHSU before or is a current employee.

All students must be accepted for a clinical placement via the OHSU exception process before completing this form.

OHA review

Student cohort requests

A Cohort is defined as four or more students attending a rotation at the same OHSU location. For cohort onboarding. please complete the Onboarding Request Form in Step 4.

Faculty requirements

All faculty members who enter the OHSU campus are required to have an OHSU badge ID.

Cohort faculty are granted building access with their badges, and must complete the following modules on an annual basis:

  1. Information Privacy and Security Essentials (current or previous year)
  2. Respect at the University
  3. Integrity Foundations
  4. Integrity Booster
  5. Abuse, Neglect and Intimate Partner Violence
  6. USP 800 Hazardous Drugs in the Healthcare Setting

IP faculty will receive a visitor badge only. 

To request ID badge access for faculty or to renew an ID badge, complete the form below. The OHSU Student Onboarding Coordinator will contact faculty using the email address provided for additional information.

FACULTY FORM

Network and Epic Information

Network login and Epic class scheduling information will be provided directly to students using the email provided in the Form above. Students will receive a secure email from OHSU.

Epic access cannot be granted until after the successful completion of training modules and begins on the requested rotation start date.
