About Care Management Plus

Founded in 2005 by Dr. David Dorr in conjunction with Intermountain healthcare, the objective of Care Management Plus (CMP) began as a technology-intensive, innovative primary care model that reduces hospitalizations, lowers costs, and improves health outcomes for older adults with chronic illnesses. The team continues to focus on interdisciplinary efforts to improve the quality of care for patients with chronic illnesses receiving care from primary care, interprofessional teams, and community-based organizations.

The CMP team consists of physicians, researchers, research engineers, instructional designers, project managers, and advisory teams. David Dorr, MD, MS, is the CMP director, in addition to serving as DICE Head, OHSU’s Chief Research Information Officer (CRIO), and co-directing the Center for AI-Enabled Learning Health Systems. LeAnn Michaels serves as the CMP team’s program manager.
