Medical scribes

Medical scribes are typically unlicensed, health-degree focused individuals who provide in-person, real-time documentation assistance for healthcare providers. The purpose of the medical scribe is to enable providers to deliver efficient, cost-effective, high-quality patient care by drastically alleviating the documentation burden of the electronic health record (EHR).

Scribes accompany the provider throughout the patient visit and are trained to record any pertinent information (patient histories, exam findings, diagnoses, and patient instructions) in real time as directed by the provider. Therefore, the bulk of the data entry duties are relieved from the provider, who are then responsible for reviewing, revising, and signing a completed visit note.
