2. Gather records:

For adult referrals, Cardiology:

  • Echo Reports
  • Echo Images
  • Previous Cardiac work-up

For Pediatric Cardiology:

  • Murmur - child >2 months
    • Most recent chart notes including chest x-ray reports
    • ECG strips (not just report)
    • Any Echo reports
    • If applicable, Event or Holter Monitors
  • Murmur - infant <2 months
    • Most recent chart notes including chest x-ray reports
    • ECG strips (not just report)
    • Any Echo reports
    • If applicable, Event or Holter Monitors
  • Murmur - newborn within 1st week must be seen within 1 week:
    • Most recent chart notes including chest x-ray reports
    • ECG strips (not just report)
    • Any Echo reports
    • If applicable, Event or Holter Monitors
    • If any symptoms, they need to be seen within 24 hours by the in-patient consult team.
    • If asymptomatic, they still should be seen within 72 hours.

3. Fax the referral and all records to 503-346-6854.

* Referral notes or forms should include:

  • Patient name, date of birth, sex, address and phone number
  • Referring provider’s name, address and phone number
  • Diagnosis or reason for referral
  • Department patient is being referred to
  • Most recent chart notes supporting the diagnosis or reason for referral
