Collect blood on filter paper card. 4 circles must be filled.
CORD BLOOD IS NOT ACCEPTABLE.
Deliver card to laboratory within 12 hours of collection.

Referred to State Health Laboratory. Oregon requires all infants to have two tests.
Screening tests for Phenylalamine, Methlonine, Leucine, Galactose, Galactose-uridyl Transferase, Toxoplasmosis Antibodies and Thyroxine. Thyroid Stimulating Hormone level is also determined if Thyroxine is low.
Specimen collection performed by hospital phlebotomists or Nursery personnel. First specimen should be obtained 24 hours after adequate feeding of baby, 48-72 hours after birth. Tests must be done on all newborns before they are discharged and should be done before they are 5 days old.
In addition, the mother is given a PKU card to bring with the infant at the time of the first follow-up visit (2-8 weeks of age). The provider will collect another specimen at that time and send it directly to the State Health Laboratory. The result of that screen will be sent directly to the provider.
