Pediatric Specimen Requirements:

0.5 mL blood in a LAVENDER top tube (EDTA); redraw if specimen clotted.

Specimens are stable for up to 24 hours at room temperature and 7 days refrigerated at 2 to 8 °C. For longer storage, specimens may be frozen at -20 °C or colder for up to 3 months.

Reference Range:

4 to 12 ng/mL

Comments:

Test performed by immunoassay using Siemens Dimension EXL 200.

Kidney transplant (in combination with Cyclosporine): 4 to 12 ng/mL
Toxic value: greater than 25 ng/mL

Interpretive Data: A range of 12 to 20 ng/mL has been suggested for liver transplant. Samples for analysis of Sirolimus should be collected 30 minutes to 1 hour prior to the next dose so that the measured concentration of drug represents trough levels. The optimal therapeutic range for a given patient may differ from this suggested range based on the indication for therapy, treatment phase (initiation or maintenance), use in combination with other drugs, time of specimen collection relative to prior dose and/or type of transplanted organ.

INR for venous thromboembolism: 2.0 to 3.0

INR for most patients with mech. valves: 2.5 to 3.5

Comments:

Draw volume is critical due to the liquid anticoagulant. Allow tubes to fill by vacuum.

  • Tubes with rubber stopper: fill to line on label.
  • Tubes with plastic (Hemogard) cap: the fill level is above the top of the label.
  • Syringe: do not remove the vacutainer stopper. Insert the needle through the stopper and allow the tube to fill by vacuum. Do not overfill the vacutainer.
  • Pediatric tubes have no vacuum. Remove the cap and add blood to the 1.3 mL mark. Do not overfill.
