Application & Submission

Applications are currently not being taken.  The next session will be held in 2015 and more information to come.

To apply, you will need the following:

  • The application form .pdf
  • Two reference forms (included in application above) to be completed by at least one teacher and one of the following: health professional, school counselor, adult mentor, coach, or spiritual leader.
  • A $50 application fee (this fee is refundable if you are not accepted or if you are accepted and receive a scholarship). For questions about the application fee, please contact your regional coordinator.

Your references must be faxed or mailed directly by the evaluator to the MedStars office at the address on the form. Your application and references must be complete by the deadline in order to be accepted.
