Pulmonary and Critical Care Medicine Fellowship

Our fellowship leadership loves all things pulmonary and critical care. We are passionate medical educators.

The guiding principles behind our fellowship program are:

  • It is a privilege to have fellows spend three years with us at OHSU.
  • Fellows are our junior colleagues who bring their own expertise to OHSU from diverse life and work experiences.
  • Fellows’ education is the focus of their time here at OHSU.

The majority of our clinical rotations take place at OHSU’s main campus hospital and clinics, and the Portland VA Medical Center. Fellows also have the opportunity to rotate at two community sites, Legacy Emmanuel Medical Center and Providence St. Vincent Medical Center.

OHSU provides unique clinical and career development opportunities. See some of the highlights below:

Clinical Training Highlights:

  • High acuity medical ICU with exposure to organ transplantation (except lung) and hematologic malignancy patient populations
  • Pulmonary sub-subspecialty clinics in ILD, pulmonary hypertension, cystic fibrosis as well as strong clinical collaboration with allergy & immunology, rheumatology, and ENT
  • Cardiovascular ICU with exposure to broad range of mechanical circulatory support including ECMO, Impella, durable LVAD
  • ECMO program with an ELSO platinum designation!
  • Interventional Pulmonary / Lung Cancer rotation at Portland VA featuring state of the art lung cancer screening and diagnostics – we have two IP-fellowship trained faculty at Portland VA and one at OHSU, with no IP fellow! This affords incredible exposure to all things IP:
    • Lung cancer early detection and treatment
    • Robotic bronchoscopy
    • EBUS
    • Indwelling pleural catheters

Career Development Highlights:

Our main goal is to foster your growth and professional identity. Although your first year will be clinically heavy, fellows have the opportunity to reflect on their passions and meet with mentors during two career development weeks. We have also created three tracks, each with tailored mentorship and scholarly training.

After the first year, PCCM fellows will choose one of three tracks to pursue, each with tailored mentorship and scholarly training:

  • Physician scientist: fellows on this track do up to 18 blocks of non-clinical time spent working closely with a mentor in clinical or basic science research. Fellows on this track often pursue additional coursework through the OHSU Human Investigations Program, which can be the foundation for a Masters in Clinical Research. We encourage fellows on this track to apply for our Pulmonary T32 training grant that can fund additional years of research after the ACGME fellowship years. 
  • Clinician educator: fellows on this track do between 21 and 26 blocks of clinical time with roughly 13 blocks of scholarly time. Clinician Educator fellows are strongly encouraged to apply for and complete OHSU’s Education Scholars Program, a one-year certificate program in education theory and scholarship.
  • Clinical leadership: this is a predominantly clinical track, with approximately 6-9 nonclinical blocks for scholarly work focused in QI. We encourage fellows to pursue a niche within pulmonary and critical care for their additional clinical time above ACGME / ABIM minimum requirements.

Conferences:

We have significantly revised and expanded our conferences in recent years.

Monday: Case Conference 12 PM to 1 PM, Tuesday: Ambulatory Conference 12 PM to 12:30 PM, Wednesday: Research Conference 11 AM to 12 PM, Thursday, Fellows Conference 12 PM to 1 PM, Rotating Conference Schedule 1 PM to 2 PM, Friday: PACCM Grand Rounds 12 PM to 1 PM

Schedules: Our ICU schedules are predominantly day / night shifts. Pulmonary consult services are daytime with alternating home call and alternating weekend coverage. Fellows have continuity clinic at both the Portland VA and OHSU, on alternating weeks.

Fellow Wellness: OHSU has a comprehensive Resident and Faculty Wellness Program. This provides everything from public speaking coaching, debriefing, and comprehensive mental health care including prescribing meds. Just about the only thing they don’t do is couples counseling.
