Clients receive long-term care coordination in relation to a variety of jointly identified issues such as housing, insurance, mental health, substance abuse, and non-HIV related health problems. Clients assessed to need Nurse Case Management are provided assistance monitoring their medication, health and coordination of medical care.

Clients can leave services when their goals have been accomplished and can also return to services if needed.

Services are provided in a culturally and linguistically appropriate manner. We have two staff members who are fluent in Spanish. We also contract with the Immigrant and Refugee Community Organization (IRCO) to provide interpreters for clients fluent in languages other than Spanish.
