How you pay for your healthcare will depend on whether you have health insurance and the type of coverage you have.


Some services need authorization or a referral before you receive care. Be sure to confirm what your insurance company covers before you receive care. You could be responsible for all charges if you do not have proper authorization or referrals.


To make sure registration and insurance billing is as convenient as possible for you, please give us complete information about your primary and/or secondary health insurance and bring your insurance card(s) with you to any visit. Let us know if your personal information has changed since your last visit.


Insurance co-payments should be paid at the time of service.
