If you would like to directly refer a patient, please fax the referral note to 503-346-6854. Records and images will be reviewed by our multidisciplinary team. We will then contact the patient with information about additional testing if recommended and to schedule appointments. For routine referrals, this process can take several weeks if there is delay or difficulty in obtaining records.
