What you need to know about student compliance and immunization requirements 

All OHSU School of Nursing students are required to provide documentation regarding the completion of their immunization and compliance records as identified in the Student Compliance and Drug Screening policy 50-01.15 and the OHSU Student Health Service pre-entry guidelines policy 02-01.001. Review the following steps to complete these requirements by the stated deadline. 

Students MUST complete ALL requirements prior to their compliance deadline. If students have not completed them by 14 days after matriculation, a $95 fee will be charged. If you are not compliant with all requirements within 30 days, a hold will be placed on your student account that will suspend your student status and prevent you from registering and attending classes.   

Steps to compliance

New matriculating students, please note: According to Oregon Administrative Rules polices (OAR-409-030-0220 background checks; OAR-409-030-0210 drug testing),  new students cannot complete a background check and drug screen more than 90 days before their educational term begins. 

New students will be receiving an email around 45 days prior to matriculation that a background check and drug screen request has been submitted for you to a platform called Advanced Reporting. Advanced Reporting is our background check and drug screen platform partner. Please be on the lookout for an email from automation@instascreen.net. You will need to complete the requested information in that email to become compliant for the OARs background check and drug screen policies. An electronic authorization form will be emailed to you which will allow the background agency, Advanced Reporting LLC, to verify the personal information you provide, and share its results with the School Of Nursing Compliance Officer. It’s important to note that once you receive the email from Advanced Reporting, you only have 7 days to begin the background check ordering process, and have 21 days to complete a drug screen from the day you receive the Advanced Reporting email with the background check and drug screen orders.

Continuing students that need an updated background check or drug screen for a clinical rotation, please reach out to the compliance officer or clinical placement coordinator for further assistance. 

Ordering a closer drug screen location for students in more rural locations:

If you are not able to locate a close testing site for your drug screen, you may try and request a closer collection site, but you will need to reach out to the Advanced Reporting directly for assistance.

NOTE: New incoming students gain access to their OHSU accounts, Compass and their network IDs around 60 days prior to their start date. It is important for students to retain the email that contains account information as that email will expire. As a reminder, new students will not have access to complete their required Compass modules until 60 days prior to matriculation.

New incoming School of Nursing students are required to complete all compliance forms, and upload them into their Exxat account once they get their Exxat log in welcome email 60 days prior to starting the term. 

Criminal history may impact future nursing licensure and impact the ability of a student to progress through their designated nursing program.  Please review the Background Check Disclosure form below and turn in the signature form as part of your incoming compliance materials.

As a reminder, new students will not have access to their Exxat account until 60 days prior to starting their program. Students will receive a welcome email when they have been granted access to Exxat.

Submit all completed documents to the Compliance Office at soncompliance@ohsu.edu.

  • Review the list of required immunizations for your program in the instructional brochure.
  • RNBS and HSOL programs are not required to provide immunizations due to being a fully online program with no clinicals. 
  • New Students will need to complete a Pre-Entrance Immunization Status Form, and send it to soncompliance@ohsu.edu along with their immunization records once they are within 60 days of starting their program. Student's immunization information will be entered into their Enterprise Health (EH) account which is OHSU's immunization and health records compliance tracking platform. 
  • COVID-19 Vaccination InformationOHSU requires immunizations for its students in accordance with recommendations from the Centers for Disease Control and Prevention (CDC) and from the State of Oregon. The School of Nursing and OHSU strongly recommends that all OHSU members including students, regardless of their work location or job function, remain “up-to-date” with COVID-19 vaccines, as defined by the Centers for Disease Control and Prevention. Under the revised policy as of June 12, 2023, all students must be “fully vaccinated” or “up-to-date” with COVID-19 vaccinations, OR they must submit a declination form through Enterprise Health, and adhere to Student Health’s current requirements designed to minimize COVID-19 transmission. New students wishing to submit a declination should do so when they receive their OHSU network login credentials (usually about 60 days before the start of your program).

    Currently, if a member or student chooses to decline the vaccination, they will need to wear a surgical or procedure mask whenever working with patients. Students must comply with OHSU pre-entrance immunization requirements per Policy 03-30-150 and Policy 02-90-010, and remain in compliance for the duration of their enrollment at OHSU. 

  • OHSU requires all eligible students to carry comparable medical health insurance. All eligible students will be automatically enrolled in the student health insurance plan (excludes RNBS) and have the student health insurance plan premium billed to their student account unless they have other comparable coverage AND complete and are approved for a SHIP waiver by the deadline. Students Medical insurance questions should be directed to OHSU's Business Office, 503-494-7617 Option 4.
  • Flu Attestation/Declination: Students taking courses in clinical agencies are required to get a flu shot prior to participation. Students that do not wish to get a flu shot can submit a completed Flu Shot Declination Form through their Enterprise Health account. Flu shot and flu declinations need to be completed no later than November 1st of every year. 
  • Possible exemptions 

CPR Certifications and RN Licenses Will Need to Get Uploaded Into Exxat and Acemapp Accounts.

  • CPR BLS Certification (required of all students except public health and nursing undergraduate students). The School of Nursing can only accept CPR/BLS certifications issued by the American Heart Association, no exceptions. New undergraduate students do not need to get their CPR BLS certification before starting their program. New undergraduate nursing students will get CPR BLS certified during your first term in one of the nursing courses. Please note that students cannot be in clinicals if their CPR/BLS certification lapses. Non-compliant students will be pulled out of clinical if it lapses. Students are responsible for maintaining an active certification at all times while in their program. 
  • For students located near the Portland campus: OHSU and the American Heart Association offers a BLS HeartCode course that is designed for healthcare professionals and other personnel who need to know how to perform CPR and other basic cardiovascular life support skills in a wide variety of in-facility and prehospital settings. The course is composed of two parts: an online module (1-2 hours to complete) followed by a 45 minute in-person skills demonstration at the Portland campus. Please note that these courses fill up quickly so we recommend that students plan ahead when looking into this option for CPR BLS renewal. Reginal campus students should reach out to their program coordinators to get details on recertification courses within your area. 
  • RN License Requirement: All Graduate level students and RNBS students are required to provide proof of an Oregon RN license (not required for Public Health, Accelerated Bachelors, or OCNE 3 year students)
  • Electronic photo requirements for PDX Graduate and RNBS students. Accelerated Bachelors and 3 year Bachelors students on the Portland campus do not need to submit this form or a photo.

OHSU requires all admitted students to provide proof of being fully vaccinated for COVID-19, or complete a COVID-19 declination through Enterprise Health prior to the start of the academic program. Students will need to inform the Compliance Officer that they are planning on submitting a COVID declination. 

All students are required to enter their COVID vaccination information into Enterprise Health (EH). If a student has received their COVID vaccination(s) outside of OHSU, they will need to enter their COVID vaccination information into an OHSU's platform called Enterprise Health (EH). Students will use their OHSU network ID to log in, but will not have access to their EH account until 60 days prior to the start of the term. If a student is experiencing log in issues with their EH account, they will need to reach out to the SON Compliance Officer directly so they can troubleshoot. If a student receives their COVID vaccinations through OHSU, then their information should already be in their EH account.

Once a student has access to their OHSU network ID and access to Compass, they will be able to log into Enterprise Health (EH). Again, this is about 60 days before starting their program.

Below are the steps that will walk students through the process on how to upload their COVID information into the Enterprise Health (EH) platform. 

  1. Log into Enterprise Health using your network OHSU ID information: https://webchartnow.com/ohsu/webchart.cgi
  2. Click on Upload COVID Vaccine Documents
  3. Upload your document and hit submit 

OHSU requires immunizations for its students in accordance with recommendations from the Centers for Disease Control and Prevention (CDC) and from the State of Oregon.
