Transplant Coordinators
OHSU transplant coordinators provide compassionate, evidence-based, exemplary care to patients, families and caregivers. They practice as part of our multidisciplinary team and embrace the core values that serve as a foundation for the delivery of quality patient care. A transplant coordinator advocates for patient safety, health and well-being.
Transplant coordinators partner with you to implement a care plan aimed at successful outcomes. Our transplant coordinators participate actively in your care, providing health assessments and health education from pre-transplant through post-transplant care.
Transplant Social Workers
Our transplant social workers are invaluable members of your transplant team. They perform services as a member of a multidisciplinary team that includes kidney, pancreas, liver and heart transplant candidates. They help in all phases of transplantation, including adjustment to illness, injury or disability, hospitalization or other treatment and readjustment to the community after leaving the hospital.
