For Healthcare Providers: Refer a Patient

Providers: please see the attached document regarding changes the transplant program is making in response to COVID-19.
Patients must be referred to Clinical Transplant Services at OHSU by a physician. We do not accept self-referrals.
Each transplant team requires specific details and medical documentation. When all of the required information is received, it is reviewed by a transplant coordinator to make sure the patient is medically appropriate and meets the criteria to move forward in the process.
If the patient is identified as a possible transplant candidate, their insurance benefits will be verified and our financial coordinator will screen them to make sure they have resources available to support transplantation. Once it is determined the patient has the financial resources they will need, we will ask their insurance carrier to authorize a transplant evaluation.
Heart transplant referrals and protcols
Heart Transplantation referrals require a physician-to-physician request.
Refer a patient
- Refer your patient to OHSU.
- Call 503-494-4567 to seek provider-to-provider advice.
Kidney, and simultaneous pancreas-kidney transplant referrals and protocols
If you are a referring provider, please complete OHSU Patient Referral Form as well as the Kidney Transplant Referral Form. We will not be able to process the referral until all requested information and documentation is received.
Liver transplant referrals and protocols
If you are a referring provider, please complete all of the information requested on the referral forms and fax to 503 494-5292. We will not be able to process the referral until all requested information and documentation is received.
