We understand that living with diabetes can be challenging and it can be difficult to filter through accurate and inaccurate diabetes "information" that is out there. Whether you have had diabetes for 20 years or more or having diabetes is brand new to you, this program can help you!

The Harold Schnitzer Diabetes Health Center Successful Living with Diabetes Program has been recognized by the American Diabetes Association for quality Self-Management Education and is facilitated by a multidisciplinary team of nurses, dietitians and behavioral health professionals.

The program includes 4 sessions and all sessions take place at the Beaverton Clinic. This program is covered by most insurance plans, but requires a referral from a medical provider.
