The Harold Schnitzer Diabetes Health Center Successful Living with Diabetes Program has been recognized by the American Diabetes Association for quality Self-Management Education and Support. Our program is facilitated by a multidisciplinary team of certified diabetes educators, nurses, dietitians and behavioral health professionals. All sessions take place at the Harold Schnitzer Diabetes Health Center. This program is covered by most insurance plans, but requires a referral from a medical provider. - Problem solving practice with real world diabetes challenges.
- Optional grocery store tour that provides hands-on practice buying healthy food.
