This presentation will highlight ongoing work to leverage data and technology to implement efforts to address social determinants of health (SDH) in primary care, support individualized care plans, and tailor clinical care decision-making. As social needs have risen exponentially during the COVID-19 pandemic, the importance of investing in evidence-based solutions to ensure we do high-quality work in this area has become even more apparent.
