The purpose of our Interprofessional Solutions: Chronic Care event is to cross-train primary care team members on metabolic syndrome, diabetes and hypertension.  We will also explore clinical tools, workflows and interventions that can be used in a primary care setting.  We will dialogue with healthcare professionals about chronic conditions in black communities, learn and practice behavioral change interventions, discuss tools to engage people in self management as well as explore how we can encourage people with chronic conditions to increase their physical activity.
