Oregon's Coordinated Care Organizations make early strides to meet OHA value-based payment goals by 2024

The Oregon Health Authority tapped the OHSU Center for Health Systems Effectiveness to evaluate the first year of the VBP Roadmap for CCOs, revealing early progress and opportunities for additional state support.

One in four Oregonians are enrolled in the Oregon Health Plan (Medicaid), and most are members of one of 15 Coordinated Care Organizations across the state.

In 2019, the Oregon Health Authority developed the Value-based Payment Roadmap for CCOs to promote the adoption of value-based contracting between CCOs and providers, and is a foundational element of Oregon Health Authority’s CCO 2.0 contract cycle and state health transformation goals. The Roadmap requirements ensure at least 70% of CCO payments to providers will be made as VBPs by the end of the cycle in 2024.

The OHSU Center for Health Systems effectiveness was selected to evaluate the implementation and early outcomes of the roadmap, and the baseline report was recently released by the OHA.

CCOs paid roughly one-third of Medicaid dollars under VBP arrangements in 2019, but faced steep challenges as they sought to accelerate providers’ adoption of VBP models while also responding to COVID-19 in 2020. The CCOs appeared on track to achieve the earliest requirements of the roadmap, but OHA can support these efforts by aligning its VBP and quality measurement priorities, providing guidance to address health disparities and exploring statewide strategies to promote VBP adoption among harder-to-reach provider organizations.
