Enrollment and Degree Verifications

We strive to fulfill all enrollment verification request in 7 business days and your request will be added to our processing queue and processed in the order received.

3rd Party Verifiers: OHSU has authorized the National Student Clearinghouse to provide enrollment and degree verifications to third parties. The National Student Clearinghouse can be contacted:

  • Online at www.degreeverify.org
  • By mail at National Student Clearinghouse, 2300 Dulles Station Boulevard, Suite 220, Herndon, VA 20171 

Active Students

Enrollment Verification Forms: Please submit your form through our Enrollment Verification Request process :

  1. Input personal information
  2. Request: Completion of the attached form & upload form in PDF format & specify term to be verified
  3. Include any additional information if needed
  4. Deliver to: Input delivery instructions
  5. Sign & Submit

Enrollment Letter: If you are in need of an enrollment verification letter, please submit one through our Enrollment Letter Request process:

  1. Input personal information
  2. Request: Enrollment Verification Letter & specify term to be verified
  3. Include any additional information if needed
  4. Deliver to: Input delivery instructions
  5. Sign & Submit

Alumni 

If you need an education verification or degree verification form filled out, please first check if the form requires transcripts or any other documents and follow the form submission procedures below. 

Degree Verification Form: Please submit your form through our Degree Verification Request process:
