Agreements
Materials Transfer Simple Letter Agreement (UBMTA)
On March 8, 1995, the National Institutes of Health (NIH), on behalf of the Public Health Service (PHS) and the Centers for Disease Control and Prevention (CDC), published the final version of the Uniform Biological Material Transfer Agreement (UBMTA). For institutions which have signed the UBMTA Master Agreement, materials can be transferred under the terms of the UBMTA upon execution of an Implementing Letter for the particular transfer. OHSU is a signatory of the UBMTA.
Consulting Statement
OHSU is not responsible for negotiating or reviewing consulting agreements on behalf of OHSU faculty and staff. The guidelines for consulting are covered in the Outside Activity/Outside Compensation portion of the Conflicts of Interest and Commitment policy

Faculty are also advised that personal consulting agreements may result in conflict of interest situations with their university employment and, as such, must be disclosed to their respective Department Chair for that determination and possible inclusion in a conflict of interest management plan.
We recommend adding the following statement to any consulting agreement that you negotiate:
