Teleforms and eForms
Teleforms are paper forms that can be used for data collection. The paper forms are filled out by hand using a pencil or pen and then are scanned. The data from the forms is then stored electronically in a database. Teleforms are usually a quick and inexpensive way to get started with robust and secure data collection. They are also appropriate for studies where information can only be gathered using paper and not a computer.
eForms are an electronic version of Teleforms (see example above). eForms are pdf files that can accept data entry. The advantage to eForms is that the data is entered electronically from the beginning, so there is even less chance of error. Also, the pdf files may be placed on a web server that can be accessed from mulitple sites on-campus and/or off-campus.
