Referring Physicians

Physicians outside the OHSU system should download the Referral and Intake forms and fax them, along with the patient's medical record to 503 494-0966.

If your patient requires preauthorization to be seen in our clinics, the Neuromuscular Diseases Center staff will gladly assist you in completing this process.

If the situation is URGENT, please note that on the Intake form AND contact the Neuromuscular Disease Staff by phone to ensure prompt attention.

All patients will need to complete the forms contained in the Welcome packet. This can be done prior to the first visit to save time, if the provider or patient downloads them, or at the initial visit.
