What is Grant County CHIP?

An alliance between health care providers and the community developing and implementing strategies to improve the local health care system and health status of area residents.

Why is it Important for Grant County?

Health improvement is both an individual and health system activity. The health of area residents is determined by both individual health behaviors and lifestyles and the availability and accessibility of the appropriate health resources.

Mission

The Community Health Improvement Partnership is dedicated to improving the health of Grant County.

Vision

Within the next 5 years, CHIP will have helped to create a more healthful Grant County by working collaboratively through active community participation to develop local innovations to strengthen and sustain our health care system, promote health and wellness, facilitate communication and coordination between health care providers and advance community outreach efforts.
