A Certificate Program for Rural Health Care Leaders and Managers

Thanks to a grant from the State of Oregon, special funding has been made available for rural health providers to earn a certificate of rural population health management. Because of this grant, this certificate program is offered to all Oregon rural health care leaders at no cost. This series was created to engage hospitals, health systems, and other providers to transition to a value-based, population health management environment. This comprehensive certificate program will provide you with proven skills, information, and tools. You will learn what has worked in other areas and how to address implementation challenges that are unique to rural communities in Oregon. The program will take place over three sessions: Sept. 27, Nov. 1, and Dec. 6.
