Population Health Portal & Introductory Webinar Recording Available

The National Rural Health Resource Center (The Center) has created a Population Health Portal in cooperation with the Federal Office of Rural Health Policy (FORHP).

The Population Health Portal is designed to help critical access hospitals, Flex Coordinators and rural health networks navigate the journey towards improved population health.

About the Portal

This Population Health Portal adopts a systems-based approach modeled after the Baldrige Framework for Performance Excellence in managing the complexities of transitioning to value-based care and payment. This comprehensive approach, which includes the ability to measure and show value, can also help rural health organizations frame the essential components for adoption of population health as an integral strategy. The Portal features a readiness assessment for hospitals and networks and connects tools and resources to the organization’s unique strengths and needs for transitioning towards population health.

The Portal introduces a data tool, created by Ryan Sandefer, MA, CPHIT and David Marc, MBA, CHDA at The College of St. Scholastica, which helps rural stakeholders learn how to effectively conduct population health analytics with access to a web-based database for acquiring public geographic health and hospital data. It includes educational modules offering step-by-step instructions of common population health analytical procedures.
