Seeking Oregon Medical Practitioners for Focus Groups on Patient Engagement and Self-Management Education
Practitioners: Join online focus groups on October 13 & 14 or October 24 & 25, 2016
If you are a licensed and practicing medical doctor, naturopathic doctor, nurse practitioner or physician assistant in Oregon whose primary role is direct patient care, you are invited to participate in an online focus group on patient engagement and self-management.
HealthInsight Oregon is facilitating two focus groups in October, each over a two-day period, through which you can share your perspectives, knowledge and attitudes about self-management education programs and the role of practitioners in supporting patient self-management. After you register, we will contact you to confirm your participation and provide instructions for using the online discussion boards. Participants are expected to sign into the discussion boards once each day during their two-day focus group.
About This Initiative
The focus groups are part of a two-year Special Innovation Project grant awarded to HealthInsight by the Centers for Medicare & Medicaid Services (CMS). The purpose of the project is to
- Capture promising practices and emerging models from communities that are working on integrating evidence-based self-management programs (ESMPs) into patient care through designing innovative closed-loop referral and funding models
- Better understand how integrating ESMPs impacts the activated patient following completion of an ESMP
- Help accelerate shared learning across communities working to achieve integrated sustainable systems for ESMPs
The products and tools developed from this grant will help to inform future CMS funding and support for improving systems that integrate the use of ESMPs. Your participation in these focus groups will help inform final recommendations for CMS.
HealthInsight is the Quality Innovation Network-Quality Improvement Organization (QIN-QIO) serving a four-state region encompassing Nevada, New Mexico, Oregon and Utah. As part of your QIN-QIO, HealthInsight Oregon works with Oregon providers and communities to promote patient-centered care, make care safer and more affordable and improve population health. We engage medical practices, hospitals, surgery centers, home health agencies, Medicare consumers and families, public agencies and other key stakeholders to develop and apply proven strategies related to prevention and health promotion.
