Oregon’s Coordinated Care Model: Highlighting Outcomes and Promoting Excellence in Implementation

The Oregon Health Authority (OHA) is hosting its annual Coordinated Care Model (CCM) Summit on November 17. This summit’s theme is “to highlight outcomes and promote excellence in implementation of the coordinated care model.” Elements of Oregon’s coordinated care model are: transparency in price and quality, shared responsibility for health, measuring performance, sustainable rate of growth, paying for outcomes and health, and best practices to manage and coordinate care. We anticipate that providers and clinicians, public and private health care purchasers, community stakeholders, health leaders, lawmakers, policymakers and funders will attend.
OHA is accepting proposals from presenters with examples of innovative work that address components of the coordinated care model. Priority will be given to submissions that address the overall summit theme, add to the diversity of perspectives at the summit, and/or include outcomes data supporting emerging and best practices.
