The OCCYSHN Care Coordination Toolkit is intended to support primary care providers in the implementation of a comprehensive care coordination program in their offices as part of medical home transformation. Join Us for the Coffee Time Consultation Monthly Series
A brief formal presentation on clinical issues for children and youth with special health needs will be followed by 10 minutes for your questions and case discussion. Presenters will be available by e-mail to answer additional questions following each webinar. There is no cost, and all that is needed is internet access and a telephone.
