Laboratory tests may be ordered by authorized health care providers using standard electronic interfaces or requisitions/forms. Ordering nomenclature should coincide with the OHSU list of tests provided in the Test Directory. Orders which are unclear, illegible, or non-standard, will require confirmation from the ordering provider/location before testing can be performed. Tests ordered which are not in the directory are subject to review and approval by the Medical Director. All specimen requirements should be followed to avoid delays in testing.

Phone orders will be accepted to add tests to a sample already in the lab, but must be followed immediately by a written or electronic order.
