Your First Visit

Before your first visit, please call 503 494-8505 or 888 222-6478 to pre-register for your appointment. This allows us to have information in our records at your first appointment. If you are concerned about payment, ask if you qualify for financial help based on your ability to pay.

On the day of your appointment, please come to the Harold Schnitzer Diabetes Health Center 15 minutes before your appointment time. Please bring:

• Your completed health questionnaire
• Health Questionnaire- MD Visit
• Health Questionnaire- Education Visit
  
• Your blood glucose meter
• Your log book of recent blood glucose readings and
• A list of medications you are taking now.

Your appointment is in the Harold Schnitzer Diabetes Health Center & Adult Endocrinology Clinic. We are located on the first floor of the Physicians Pavilion, Suite 140. Please park in OHSU patient parking, in the garage under the Physicians Pavilion.

If you cannot come to your appointment, please call 503 494-3273 or 888 222-6478, extension 43273, as soon as possible. Please call at least 24 hours before your appointment time.

Directions and Parking

Driving to OHSU: Please call the OHSU Directions Line at 503 494-1390 for automated instructions on driving to the OHSU Marquam Hill Campus from around the Portland area.  To Learn More

Parking: Free patient parking is available in the parking garage directly below the Physicians Pavilion. Free valet parking is available Monday through Friday from 7 a.m. to 6 p.m.

Taking the bus: The TriMet No. 8 bus to Jackson Park coming from downtown Portland goes to the OHSU Marquam Hill Campus. Take the No. 8 bus labeled "To Jackson Park/Veteran's Hospital" heading south from the downtown transit mall.
