We accept most medical insurance plans. If you wish, you may also pay for your treatment directly.
If you are new to our office, a referral from a doctor and authorization from your insurance company is required before your first visit with a doctor or diabetes educators. Use this form to start the referral process:

If you do not have medical insurance, or will pay cash for your visit, we will ask you for a deposit (payment) at your appointment.

New to OHSU?
If you have not seen any OHSU doctor or are new to the Harold Schnitzer Diabetes Health Center, we need a doctor's referral. Please have your doctor complete the Physician Consultation Request Form and fax it with supporting medical documents to 503 418-2208. Once we receive your referral, a staff member will contact you to make an appointment.
Already seeing an OHSU doctor?
Ask your doctor to submit an electronic referral (Consult to Diabetes Center Adult). Once we receive your referral, a staff member will contact you to make an appointment.
